Treatment Consent Form

Please read the statement below and confirm your consent before your first appointment.

Healing as a therapy complements any other treatment you are receiving, and your GP should be consulted about medical conditions that may need attention.

I understand that the treatment offered is complimentary to any other prescribed treatment; it is not a replacement for medical treatment. I have had explained to me the processes involved, whether the treatment is Hypnotherapy, Reiki, a combination of both or Past Life Regression, and understand and consent to the treatment. Once the treatment has been delivered and paid for the cost is not refundable. I am over 18.

Your confidentiality will be respected at all times, with the exception of:

If you, or the person you are signing on behalf of, is in danger of causing serious bodily harm to themselves or others;
Suspected child abuse.

Consent must be given in writing using this form before your first appointment. Your personal information is protected under UK GDPR: it is stored securely, kept confidential and used only for your therapy records.
